Diet Gallstones

Diet can have a major influence on the development of gallstones. High-fat diets, particularly saturated fat, and overconsumption of refined carbohydrates can stimulate gallstone development. Ample dietary fiber and moderate intake of alcohol decrease the risk. Being overweight sharply increases risk of gallstones, whereas weight loss in obese persons can cause chronic stones to dissolve and clear.10 In individuals with gallstones, consumption of fatty foods or coffee can bring on painful gallbladder spasms. Food sensitivities are often an unrecognized trigger of gallbladder symptoms - eggs, pork, and onions are the most commonly implicated.

Phase Three Complications to Months

For a minority of patients, this phase is characterized by difficulty in adjustment to weight loss, food intolerances, self-destructive behaviors, and, occasionally, requests for a reversal of the procedure. These patients tend to be time-consuming for the treating physician. Discerning the difference between psychological symptoms and physical symptoms can be difficult in this phase. Gallstone formation is associated with rapid weight loss,20 and the reported prevalence of gallstones at six months postoperatively is 22 percent.21 Small-bowel obstruction from internal hernias after laparoscopic surgery13 can present with crampy abdominal pain, nausea, and vomiting (Table 8.4). A high index of suspicion and close consultation with the bariatric surgeon is needed.

Use in Prevention and Therapy

Supplemental taurine can enhance bile acid function and reduce the likelihood of cholesterol-type gallstones.1 Taurine can also enhance fat and fat-soluble vitamin absorption from the diet, particularly in people who have difficulty absorbing fats due to liver, gallbladder, or pancreatic diseases.8

Diseasespecific Guidelines

The prevalence of overweight and obesity is reaching epidemic proportions nationally and worldwide.75,76 In the U.S., 55 of adults aged 20 years and older are either overweight or obese. Overweight is defined as having a body mass index (BMI) of 25.0 to 29.9 kg m2 and obesity as BMI equal to or greater than 30 kg m2. Similar trends are observed in children and adolescents with a doubling of the number of obese children from 20 years ago.75 Obesity is a major risk factor for coronary heart disease, hypertension, type 2 diabetes, stroke, gallbladder disease, osteoar-thritis, respiratory problems, and some cancers. In addition to the enormous health consequences that result in increased health care costs, there are significant social and psychological burdens for obese individuals and their families.77

Myocardial Infarction MI

MI is characterized by crushing chest pains that may radiate to the left arm, neck, or upper abdomen (which may feel like acute indigestion or a gallbladder attack). The affected person usually has shortness of breath, ashen color, clammy hands, and faints. Treatment within one hour of the heart attack is important and usually includes chewing aspirin and administering CPR. Many individuals die each year of their first MI.

Etiology and pathophysiology

While the majority of adult cases of pancreatitis are related to either alcohol consumption or gallstone disease, the causes of pediatric pancreatitis are more varied. In adolescent females, gallstone pancreatitis is not uncommon. However, young children with this disease have often been exposed to a recent trauma, infection, or medication as the inciting event. Recurrent pancreatitis in childhood can be attributed to hereditary pancreatitis, an anatomic variant of the pancreatic or biliary tree, or an underlying systemic or metabolic disorder. Changes in pancreatic size, contour and texture are best appreciated with ultrasound. This modality is also excellent at identifying ascites, abscesses, pseudocysts, dilated ducts and gallstone disease (Figures 21.2-21.5).

According to the World Health Organization, obesity has become a worldwide problem that has significant effects on health. Problems that were once considered limited to developed or industrialized countries now affect everyone. Because of obesity, the incidence of diseases such as heart disease, type 2 diabetes mellitus, and hypertension has increased around the world. Obese individuals are also prone to pulmonary disease, varicose veins, and gallbladder disease. They have an increased risk of breast, uterine, and colon cancers.

Protein Differences Between Omnivores And Vegetarians Or Vegans

A varied diet based on plant proteins is adequate, yielding growth and body maintenance results equivalent to a diet based on meat protein.39 The lower incidence of obesity, constipation, lung cancer, hypertension, coronary artery disease, type 2 diabetes, gallstones, reduced risk of breast cancer, diverticular disease, colon cancer, calcium kidney stones, and osteoporosis appear to be obvious advantages particularly of the well balanced vegan diet for the elderly.3,39,40 Key et al.,40 (Table 11.1) show the protective effect of daily fresh fruit intake in ischemic heart disease, cerebrovascular disease, and lung cancer, and daily raw salad protection for ischemic heart disease. They also presented a higher incidence of breast cancer in the vegetarian women, but the confidence interval was broad. Recommendations for OWOB Individuals with Type Diabetes and Those At Risk for the Disease

Pharmacologic treatments, including insulin (20), as discussed in the chpater by Lien and Feinglos in Section II of this book. In patients with longstanding disease or pronounced pancreatic P-cell dysfunction, moderate weight loss may not be sufficient to achieve satisfactory glycemic control. There may be a need for more intensive, ongoing intervention, including an escalation of calorie restriction to a very low-calorie diet, e.g., 800 Kcal day or less (4, 55). In this event, a routine vitamin mineral supplement and routine nutritional monitoring should be employed, and indications of medical complications, including gallstones (56), should be monitored. Weight-loss-promoting medications may need to be considered in some cases when calorie reduction and exercise do not result in sufficient weight loss, i.e., BMI > 30 or BMI > 27 plus OW OB-related comorbid conditions. Bariatric surgery may be considered with a BMI > 40 or BMI > 35 plus comorbid conditions (47).

Diet Obesity

Many popular weight-loss diets are unscientific and can be hazardous. Unbalanced diets that severely limit foods and are deficient in many important micronutrients2 should be avoided. Although high-protein diets produce rapid initial weight loss, the loss is transient because water is lost, not fat. High-protein diets can also cause kidney damage and a life-threatening fall in blood potassium. Very low-calorie diets are those providing about 300-400 kcal per day. They should only be used in selected cases and must be carefully supervised by a doctor or dietitian. They often have harmful side effects (e.g., they may trigger gallstones) and are deficient in important micronutrients.

Future trends

The prevalence of overweight and obesity is increasing steadily in many developed countries. In the UK, over a quarter of the population are either overweight or obese. Obesity is a risk factor for many conditions. During a 10-year follow-up study, the incidence of colon cancer, diabetes, heart disease, hypertension, stroke (men only) and gallstones increased in line with the degree of overweight among adults (Field et al, 2001). Thus, reducing the incidence of overweight and obesity is a major public health priority. A positive energy balance is the cause of practically all cases of overweight and obesity. Factors regulating food intake are hunger, appetite, satiation and satiety.

Vegetarian Diets
