In this short review, I have attempted to cover most industrial plants that are commonly used as dietary supplements. There are, certainly, many more such materials. However, it is almost impossible to limit the growing number of plant materials currently used to enrich food to provide health benefits. Therefore, I conclude with the prophecy that the future of plant-based dietary supplements looks very bright and that many more new materials in the form of plant extracts and essential oils will enter the global marketplace within the next decade.

Dietary supplements in the United States include a wide array of ingredients and products that are broadly available in the marketplace. While these products are presented in formats that give the appearance of standard over-the-counter (OTC) medications such as aspirin, their manufacturers are not required to provide the same level of premarket safety data as manufacturers of OTC products. In glucosamine sulfate the anion is sulfate, in glucosamine HCl the anion is chloride, and in N-acetylglucosamine the amino group is acetylated, which results in a neutral charge. All forms are water soluble. Nutritional supplements are usually derived from marine exoskeletons with the chitin extracted from seashells. There are also synthetic forms. Since glucosamine falls under the 1994 Dietary Supplement Health and Education Act (DSHEA) and is classified as a medicinal product, its manufacture is not regulated. Arginine and Immune Function Animal Studies

Barbul and co-workers demonstrated that supplemental dietary arginine reduced trauma-induced thymic involution, lessened weight loss, improved wound healing and prolonged survival in injured rats (see Barbul, 1977). The same group also demonstrated that a dietary supplement of 1 arginine by weight increased thymic weight and the number of thymic lymphocytes in normal healthy mice and rats (see Barbul, 1986, 1996). In a burn model in guinea pigs, Saito et al. (1987) showed an increase in delayed hypersensitivity and in survival in arginine-supplemented animals. Madden et al. (1988) demonstrated a similar survival advantage in animals subjected to lethal bacterial peritonitis. This finding was replicated by Gianotti et al. Other potential uses of immunoregulatory probiotics (e.g. in controlling inflammatory diseases at the gut surface) have only recently begun to attract research attention (Venturi et al., 1999), partly because the microbial gut mucosal signalling mechanisms are only beginning to be understood by microbiological researchers (Haller et al., 2000). A recent pilot study (Gupta et al., 2000) showed promising preliminary results for the use of L. rhamnosus GG as a dietary supplement to reduce clinical indices of GI-tract inflammation in children with Crohn's disease. As research starts to define the interactions of the gut microflora and the immune system in the maintenance of health, so it is likely that new avenues for dietary intervention will become the focus of research efforts.

Weight Loss Drugs and Supplements

Orlistat (Xenical) is a lipase inhibitor that decreases fat digestion in the small intestine by blocking the action of lipase coming from the pancreas. This in turn decreases fat absorption which can support weight loss. However, the decreased fat absorption means that more ends up in the large intestine and thus in feces as well as acted upon by the bacteria in the colon. Side effects can include frequent oily bowel movements, diarrhea, bloating, and abdominal pain. Alli is the dietary supplement version of Orlistat and provides the same active ingredient but at half the level of Orlistat. It was released to market in mid 2007 with guidance to consumers to use a low-fat diet to reduce the risk of undesirable side effects.

Regulations Related to Functional Foods

Functional foods are regulated by the United States Food and Drug Administration (FDA) under the authority of two laws. The Federal Food, Drug, and Cosmetic Act (FD&C) of 1938 provides for the regulation of all foods and food additives. Effects of conjugated linoleic acid on body composition

In men who were supplemented with 3.4 g day of a CLA mixture, purified t10c12 CLA isomer, or olive oil placebo, the t10c12 CLA isomer exerted an increase in insulin resistance that correlated with increased urinary isoprostane levels, suggesting an increase in oxidative stress in these same individuals. Previously, in a study from the same group, 60 abdominally obese men supplemented with 3.4 g day of the t10c12 CLA isomer, a CLA mixture, or equal amounts of olive oil became more insulin resistant when supplemented with t10c12 CLA than did people supplemented with the CLA mix or olive oil control.27 These results are significant in the clinical usages of various isomers of CLA as a dietary supplement.

Pharmacological Diet Aids

Herbal dietary supplements should be avoided during pregnancy due to the lack of clinical trials investigating safety and efficacy 4 . Herbal and botanical products are not regulated and therefore can contain a number of compounds that may be unsafe for consumption during pregnancy. Of particular concern would be herbal dietary supplements marketed for weight loss. Mind-body interventions recognize the connection between the physical body and the spiritual self, and include practices such as meditation, prayer, and music therapy. Biologically-based modalities are primarily nutrition-related and vary from special diets such as the macrobiotic diet to the inclusion of dietary supplements in the diet. Body-based methods involve hands-on manipulation of the body, and include such modalities as massage and chiropractic. The energy therapies are based on the concept that the body has an energy field that can be manipulated to promote healing. Drugnutrient interactions

Interactions of glutamine supplements with other nutrients and dietary supplements are not known.50 Since glutamine is metabolized to glutamate, which can act as an excitatory neurotransmitter, the potential for antagonizing the anticonvulsant effects of certain epilepsy medications exists. 50 Furthermore, since glutamine is metabolized to ammonia, it can potentially antagonize the antiammonia effects of lactulose.50 Although the aforementioned interactions are theoretically possible, no known adverse effects have been reported with glutamine supplementation. Glutamine supplements, especially the powders, should not be added to hot beverages because heat can destroy the amino acid. The FDA's new supplement labels must list all ingredients. The label for vitamin and mineral products must give you the quantity per nutrient per serving plus the DV (percentage daily value), the percentage of the RDA (Recommended Dietary Allowance). The listings for other dietary supplements, such as botanicals (herbs) and phytochemicals (see Chapter 12), must show the quantity per serving plus the part of the plant from which the ingredient is drawn (root, leaves, and so on). Fortunately these berries are available in appealing products with low toxicity thus, consumers may be willing to consume them before research validates potential benefits.

Ethical Considerations

Performance and it would be difficult to ingest enough food in the diet alone to creatine load, that it is unethical to do so. Others argue that if you allow athletes to take creatine, they may be more predisposed to try other dangerous supplements or drugs. Calcium carbonate antacids can be used as calcium supplements. They're nutritionally sound and generally cost less than products designed solely as nutritional supplements. Warning Not all antacids double as dietary supplements. Antacids containing magnesium or aluminum compounds are safe for neutralizing stomach acid, but they won't work as supplements. In fact, just the opposite is true. Taking magnesium antacids reduces your absorption of calcium, and taking aluminum antacids reduces your absorption of phosphorus. Questions arise about the role of vitamin supplements to enhance heart health. Living healthfully could be so much easier if we could just take a pill that could compensate for both suboptimal eating and suboptimal genetics. Vitamin B and Homocysteine

Dietary intake of vitamin B12 can be low in both vegetarians and veg-ans.58-60,86 The sources of vitamin B12 in a vegetarian diet are dairy products, eggs, fortified foods (especially meat analogues, soya milks, yeast extracts, and breakfast cereals) and vitamin supplements. If none of these sources of vitamin B12 is regularly consumed, intake of this vitamin will be low and the risk of deficiency will increase. More data are needed on current intakes of vitamin B12 in vegetarians, and on whether this is a determinant of homocysteine and CHD risk in these populations.

Regardless of the approach for creating a dietary energy deficit, in the case of WR for individuals with type 2 diabetes, the advice of a nutritionist registered dietitian should be sought for the design and individualization of the dietary plan. In these patients, maintenance of a healthy body mass may require long-term calorie restriction to some degree even for weight maintenance. So, it is important that the diet plan does not limit the intakes of essential nutrients (e.g., protein, vitamins, minerals) and that the diet provides a wide variety of nutritious foods in the long term. When these results are expressed as an effect size, a method of data standardization, HMB resulted in a significant effect size of a net lean mass gain of 0.15 and strength gain of 0.19 (Figure 12.2b and Figure 12.3b). Nutritional Ergogenic Agents

Taking performance-enhancing supplements is a personal choice.
